New Patient Information

When you come in for your first visit to our office you will be welcomed by our warm and friendly staff. Our receptionist will ask you to fill out our patient registration forms and she will go over your insurance information with you.

Dental Insurance Card

If you have dental insurance, please also be sure to bring your dental insurance card with you. Please be aware that this is usually a different card than your medical insurance card.
